What do you do to stay safe?

Today I’m going to the hospital again for another meeting with the CPN. I’m very scared of going after I lost it there on Tuesday. I think I’m scared what will happen, scared of losing it again, ashamed about what happened and still feeling very out of it, although not in the way I usually am when I dissociate. That gives some kind of protection. This is raw at the same time as shaken and disconnected.

Also I’ve got an inescapable question that has been in my mind for several weeks. I’m not at all stable or safe at the moment. I want to continue with therapy. I committed to the group that I’d do it and not give up. I promised to God and Mother Mary in prayer. I’ve made quite a few sacrifices for it – I don’t think I’d have had to leave my last job if it weren’t, at least in part, for my therapy appointments (though my last employer were definitely at fault too, in my opinion). I’ve seen the therapy as the only hope of learning how to get better and manage my condition. I’m privileged to live somewhere MBT is actually available (there aren’t specific PD services in all areas of the UK). I really don’t want to have to stop therapy.

However, at the moment I’m actually more unstable, at least in part because of the therapy and the emotions, memories and questions that it raises. This isn’t necessarily a bad thing. Other people tell me they can see positive changes in me, for example communicating more clearly about emotions and things that happened to me in my childhood, none of which I can yet see for myself. However I trust the people who tell me this and think it has to be a good thing. It’s another thing I don’t want to waste.

So the big question is, what to do.  I can’t keep myself safe at the moment. For example I’m “coping” by cutting, taking overdoses or higher than prescribed doses of medication, drinking* (and this really isn’t me, I do not enjoy drinking in this way), escaping from daily life by ignoring letters, calls, etc and not able to keep on top of the basics of looking after my home and myself (cleaning, cooking etc). I’m more unstable in my moods, especially anger, and I’m struggling more to hide everything to try to participate in daily life by eg going to work. Things like hallucinations or paranoid thoughts or feeling dissociated are pushing their way more into the working day.

I don’t know what to do to change this.

I’ve some hope that medication changes could help and I’m seeing the psychiatrist on Friday. But I doubt that’s going to be the only answer. I’ve tried to exhaustion (both daily and when in crisis moments like the extreme distress or wanting to end everything) the techniques I know like distraction and grounding and self care / self soothing (this latter is very hard for me to do when I feel as I do about myself). It isn’t working. And I feel that the things other people could do to keep me safe, many of which are on my crisis plan, are not happening or not working either. I’m experiencing more and more let downs where xyz help is promised then doesn’t materialise (appointments canceled, calls not returned, planned sources of support withdrawn, mistake after mistake, discharge plan not followed). Or I’m told that the help I want to keep safe doesn’t exist or I don’t qualify. What is offered – and don’t get me wrong I’m grateful that it is offered and I know it’s more than many other services provide – is not enough to keep me safe. For example when I’m suicidal a 5 minute telephone call may calm me a bit for a few minutes but an hour later in usually feeling worse than before and – this is key I think – still on my own trying to cope.

What do I do in this position? Are there other techniques I can learn to cope better? Are there other or higher doses of medications? When I so so much feel I am not safe on my own and really need someone with me (especially when I’m really distressed but also day to day because the slightest thing, as little as a letter that makes me panic or a canceled appointment,  can thrown me into extreme distress, self harm etc) what can I do? The PD service are adamant I mustn’t be admitted and don’t qualify for any carer help and ongoing support in person isn’t possible. I haven’t any other way of getting that kind of support. I live alone, my dad and step mum live hours away and I don’t have friends very locally or whom I see regularly.

So how do I do my therapy and stay safe as well? How do I either answer this need not to be on my own when I’m so much at risk and unstable, or what solution do I have to learn instead?

What do you do to stay safe between therapy appointments or between times you can access support?

I know this probably sounds silly and I do get a lot more support than most people and all I’m talking about coping with is simple daily life. Right now this is where I am.

*just to be clear, I’m not diagnosed with any alcohol problem and I’m not comparing my struggle with that of someone who is struggling with alcohol or other substance use. That is a much more painful place. I sometimes use what is probably an objectively average amount of alcohol taken with my tablets to make myself fall asleep when I can’t cope. Not a great thing to do but I’m not trying to compare the two.

Group and no more trust

Tomorrow is MBT (mentalisation based therapy) group. I don’t know whether to go.

All trust I had in the service has gone. It’s been completely wiped out by the lies and let downs of the past months, the proofs they don’t believe me, the tricks, the cuts that open me more and more vulnerable then leave me with nothing and noone.

I don’t really want anything to do with a service that does this, but I’m desperate and have nowhere else to go. I’m desperate for help but it’s denied, it’s promised then withdrawn, or I’m deemed not in need or not believed. I want to do the therapy but I can no longer go forward safely with it. I cannot cope between sessions except by overdosing to black everything out and self harming to punish myself, temporarily quiet the voices, temporarily be something other than the utter pain. I’m not allowed any of the things that would keep me safe between sessions.

If I go tomorrow, I will be so angry. I can’t say I’ll keep it under control in the session. I can’t say I’ll stay “stable”. Nothing keeps it in anymore. I can’t mentalise like this and really I don’t want to. These things just are as they are. There’s nothing to be “curious” about or explore my feelings or someone else’s thoughts. Their thoughts have been made totally clear – they don’t believe me, I’m not allowed help, they’re tricking me, they’re cutting me open then leaving me and finding more and more ways to do it. My feelings are exploding and total. Fury. Hurt. Trapped. Over the edge. Liar. Fake. Fraud. Pain. Screaming.

If I go to group I can’t avoid it being clear I have no hope and no trust left. If everyone or anyone else does trust the service and does believe they’ll help them, then maybe that gets them through and helps them and is a lifeline for them. I don’t want to destroy that.If I say what’s happened to me, even in the last couple of days, I could destroy it.

I could go and just try not to talk about anything to do with me and just be there for other people and listen to them and try to mentalise about what other people bring. But I’m so far gone over the edge I don’t think I can trust myself not to explode.

When the group started committed to do it all. I committed to not leaving. I committed it to everyone in the group – not out loud, we didn’t do that, but in my head I did. I promised to God and Mother Mary too. If i leave I break my commitment to everyone, not just the service. I really don’t want to do that.

Yet at the moment I’m just ending up in more and more danger. It seems as if I should just accept this’ll never end, dissociate as much as possible, hope for something sometimes bearable…. but I think I’m too far gone for that. I wish I’d never trusted them.

Utter betrayal and a sick joke

My friend has left me. Hates me and thinks I’m a selfish demanding b*tch making drama over nothing.

My PD service have basically chucked me. They’ve been playing a cruel game for a while. I beggedand begged them for help again today. I am not safe. I cannot go on. I’m cutting daily and overdosing more than every week. I cannot get a single simple little thing promised to help me even a phonecall. The game of deceiving me and shoving me nearer the edge has gone on for a while now and thoroughly broken me and ripped the wounds apart.

Now I’m utterly unable to go on and falling over the edge they will not catch me just watch me fall and laugh. I begged and begged for help. I won’t detail everything here because it would cause distress but I made it clear if I do not get help – and I need hospital or someone with me all the time now – I will seriously harm myself as much as I can and can’t be sure I won’t harm others because I’m utterly out of control.

It became crystal clear they don’t believe me and think I’m a liar and a fake and doing it for attention and don’t need help and don’t really feel these things. That was the final twist of the knife. I had known all along they thought that really but today was the ultimate proof. Nobody thinks you need hospital, they said. It’s fine for you to go. Let’s leave it there. No help. .. I want you to ring me when you get home when you’re going to take the overdose, he said. Why on earth would I do that? I had spent 2 hours telling them that’s what I’m going to do and begging for help and they didn’t help me and sent me off alone. Why on earth would I phone them to go through all that again?

Do they literally just not believe me and think I’m faking or do they think I’m such an evil bitch that I deserve this punishment and pain and they hope I do it, hope I die?

When I tell them what it’s really like and beg for the help I need they think I’m a fake. So that’s the truth. I’m evil sh*t and I do deserve hell.

I think, therefore I am, as the saying goes. ..

[Sorry. I know this post makes little sense. It’s a mess of thoughts in my head tonight since I realised how much I “am” what I am experiencing and feeling and cannot stand it and get lost along the way. ]

I think…

I feel…

It’s harder than you’d expect to separate thoughts and feelings. Thoughts can hurt. Thoughts are (must be?) quickly judged. Thoughts desire; thoughts need and long and that again is judged at once, answered or unanswered, and that brings feeling.

Can thoughts be stopped? Those that come unbidden, spiraling or shouting and yet never to be trusted, tell me I am deceiving, tell me – too bad to write… memories…

And feeling. Nothing. Terrible nothing with something clawing at me to come back, or blessed numb. Or everything.

Everything. Everything I am. All I am – pain, hurt, need, frightened – everything I am, all I am. Can’t anyone else see? Can’t you see? Everyone is in so much pain. Feeling it and absorbing it – theirs or mine? But it hits me like a wall and it’s all there is that moment, separated from time, not knowing what to do or what to be but – pain.

I think, therefore I am? I’m not sure about that! But I feel, therefore – I am not. My self, my certainties, are lost and all I’ve become is the feeling and the fear.

What do you hold onto in the darkest times?

I’ve posted before about how, like many people with Borderline Personality Disorder, one of the things I find hardest when I feel really bad is to hold on to any knowledge that it will not always be this way. The overwhelming emotions – especially fear, sadness, loneliness, anger, pain, frustration, self hatred, self disgust, hurt, distress, longing or needing, or the feelings I can’t yet name that come with flashbacks – they eclipse everything else and become all that exists.

I wonder if their power is greater if I fear the emotion I sense. But the totality of the experience, their consuming nature, makes them the more frightening.

Descriptions of this emotional experience in BPD often term the feelings intolerable or unbearable. It is that but it isn’t quite either; it’s not all of it. Intolerable, more than I can stand, yes… but it’s not something I can’t stand because it’s me. In that state there is nothing but the emotion and there is nothing of me but the emotion. I cannot stand it but neither do I exist apart from it.

I hate it so I hate myself. I must get rid of it, purge it, so I must get rid of myself and cut away the bad – so I cut.

I can name some of the emotions afterwards. Maybe the therapy is helping me to do that. But in the experience, I cannot. I cannot recognise anything but hurt and pain and hate and evil (me); I cannot hold in mind anything but the impulses to cut, run, scream, end it, reach back for numb. .. and I am gone. ..and I spin between cut off and unable to feel and any attempt to engage being painful, and the state of total emotion, of only existing as that pain.

I cannot control it. I cannot bridge that gap. Therapy is helping me identify what feelings are. But it doesn’t separate them from me, from time, from permanent reality, from right and wrong. It doesn’t tell me how to feel, rather than be, the emotion. It doesn’t tell me how to bridge the gap between the different people I become – the cut off numb one;, the one that hides everything to cope day to day and do what I’m meant to and fulfil my responsibilities and pretend and hope I could ever be good but knowing all the time that everyone really knows how fake it is and how evil I am deceiving everyone; the frightened needing child; the angry, vengeful and impulsive one. More and more they seem to be separate personalities. I am fragmenting. I am more unstable. I lose more periods of the day – when I’m in one state I cannot “access” the other and I can’t remember things that happened (though I may remember the state). I flick so quickly between states without being able to engage my rational mind and try to employ any grounding techniques or DBT techniques to control my behaviour or my experience.

I guess it’s good that I can start to be curious about the process, from the temporary relative stability of my “coping” state. It must show I do have some ability to learn to mentalisa about what’s going on in my mind. Usually my “coping” state would be trying to suppress what I’m exploring right now. Perhaps eventually I’ll be able to build a more curious and stable personality at least alongside these others.

What do you hold on to when your whole reality, your whole existence, is unbearable sensation and emotion? It sounds utterly stupid. It sounds utterly out of proportion. It sounds self centred and I am forced to admit that though it’s the very last thing I want and one of the things I most hate in myself, in a way it is, though at the same time self has got totally lost in the feeling and emotion coming from everywhere.

What do you hold on to when you can’t access your coping strategies or even your most rooted beliefs and deepest cares? I love my God and know God is mercy and compassion, but in the bad states I can only conceive of a vengeful God or a God casting me out. I love my godchildren, I care about keeping my commitments at work,  but in those states I can conceive only that I do everyone harm and everyone knows I’m bad really and would rather I weren’t around. The centre of my beliefs and values warp according to the state I’m in.

Out of it

I’m going between boiling anger that I can’t stand (a force rising inside me which I can’t swallow down, just force and power uncontrolled and bursting free) crushing anxiety with spiraling thoughts, lists, growing out of control faster than I can count, no air to breathe, dread that I can’t surmount, and numb.

Numb. Nothing. Stopped. Watching. Un-engaged. Dumb. Deaf. Hearing what everyone else says but it makes no sense and causes unbearable sensations if I try to respond – I need numb.

I’m drinking tonight to make sure I stay numb and make warmth and cotton wool replace the ache, distancing the hurt from my dissociated state so it can grow without sensing the raw pain or maddening and crushing demands of the ‘other’ (real) world.

The pain from my gynae problems has been scary too, as well as the arthritis. It’s almost funny – completely messed up inside and the physical stuff out of control too, things ‘breaking’ one after the other. Nothing medically serious but it does seem to make me as useless as possible in the real world.

I don’t often drink and it’s a dangerous and stupid choice, especially now. I’m in a really dangerous state right now. I tried and couldn’t get help. I can’t choose rationally what to do. I’m saying it’ll just be tonight and tomorrow I’ll try to face it all again.

Scared of imagining

My imaginary world used to be safe. My escape and my protection. I’d call it up at first and hide myself in it.

Later it came unbidden, with an invitation to slip through the ‘door’ to safety.

Later still, I just suddenly find myself there. Protected and dissociated. I’m safe and maybe others are too. Usually I know I’ve gone there and can hold on to an imaginary line and fight my way back into the present world.

But lately I dissociate into a place I don’t like where there’s anger and hurt and uncontrolled expressions of needs left wide, raw and empty.

And another where I’m the scared and frightened child and I’m back THERE again.

The latter two I can’t choose to leave. I have to wait til they eventually chuck me back out. I am terrified of who I am in them and how they hold me.

The lines between the worlds are blurring with more and more of my hallucinations and flashbacks. The scared child world is flashing into the present.

Knowing that as a child I could enter, at will and without will, the alternative worlds, I’m scared that other things are also false. That things I thought were in the present world then, are actually not true. That this means the awful frightening things – the abuse – things my mother did and said – did not really happen. What if I’ve somehow imagined it happened? And it didn’t really and I’ve invented it? What if I was a horrible sick evil child? Or what if I was psychotic?

So much that happened when my mother and I were alone means it can’t be corroborated.

What about the things I’m sure happened when my father was there or that I told him? And that he now says he didn’t know?

I do not know what the answer is I just know it could mean I’ve done something terrible I can never make up or make right or hurt myself enough to punish myself for.

How ever do I get an answer?

Walking this Borderland #1: Introduction to the “Walking…” series

I’ve decided to start a new series which I’ve called “Walking this Borderland”. I’m going to try to make each post in this series short and readable. My idea is that each will share an idea, skill, or thought that I find helpful in coping with an aspect of the symptoms of my Borderline Personality Disorder. Some of these are things that have been suggested to me by health professionals. Some are ideas a friend (perhaps who also has BPD) has given me permission to share. Some I have come up with or encountered myself in my path living with BPD.

I am sharing these in the hope others may find them interesting or helpful. Perhaps if you suffer with BPD or another Personality Disorder or know someone who does, you may find they are things you can identify with or are relevant or helpful to you. Perhaps they might equally be helpful to people who struggle with other mental health conditions – or even to anyone curious about emotions. Perhaps as a reader you would like to share your own experiences and ideas that help you, in the comments. I’d love it if you did want to do that.

As I have said many times before on this blog, what I’m sharing is personal and every person is very different in what is helpful to them or how they experience emotions. I really hope there is nothing I post in this “Walking…” series that would be unhelpful to anyone reading but please bear in mind that I am only sharing from my experience. Though I have worked in many mental health treatment settings and had some non-clinical training, and receive therapy myself, I am not a doctor, I am not clinically trained, I am not medically qualified to provide support or help to people with a mental health condition. So whilst I hope that this series is going to be useful, I very much urge you to please please access and rely on support from clinicians who are trained to help you.
